Smoking or an oxygen machine issue may have sparked the deadly fire at Gabriel House Assisted Living Facility in Fall River, Massachusetts, a state fire marshal said.

The Bristol County District Attorney’s Office released the previously withheld name of one of the 10 victims killed in the July 13 fire.
